NAD+ Is the Empty Bus, NADH Is the Bus Carrying Electrons

Every route your body uses to make ATP runs on NAD+. When glucose is broken down outside the mitochondria, when the Krebs cycle strips what is left inside them, and when the electron transport chain turns those electrons into usable energy, NAD+ is the carrier that picks the electrons up. Think of NAD+ as the empty shuttle bus and NADH as the bus with passengers on it, driving electrons to the power plant. If there are not enough buses, the plant runs below capacity no matter how much fuel you eat.

NAD+ has a second job that has nothing to do with energy. It is the raw material for the enzymes that repair your DNA and for the sirtuins that regulate how your cells respond to stress. Those enzymes consume it. So NAD+ is being spent on two fronts at once, and the supply has to be rebuilt continuously from what you eat and from recycling.

The Decline
Less NAD+ With Age, and an Enzyme That Burns It Faster

In human skin samples from 49 people aged newborn to 77, NAD+ fell steadily with age in both men and women while DNA damage climbed. In men the repair enzyme PARP grew more active as NAD+ dropped, because that enzyme runs on NAD+ and eats more of it as there is more damage to fix. On top of that, the enzyme CD38 rises with age, and CD38 is one of the main things degrading NAD+ in your tissues. It is also the main enzyme that breaks down NMN itself. So the decline is a supply problem and a consumption problem at the same time, and I will come back to why that matters for how you use this bottle.

What Happens to the NMN You Swallow

NMN is one step before NAD+ in the assembly line, and it does not walk into your cells as NMN. Isotope tracing shows it is trimmed to NR outside the cell and then carried in, where it is rebuilt into NAD+ with an enzyme called NRK1 as the rate-limiting step. Picture the Lego Movie: they take the thing apart, all the little workers carry the bricks through the door, and they put it back together on the other side. That is what your cells do with every precursor you take, and it is why injected NAD+ ends up going through the same door, because the whole molecule is too big to pass.

The gut adds a second route. Bacteria in your intestine convert a substantial share of oral precursors to nicotinic acid, the niacin form, which your liver then uses to build NAD+ down the niacin pathway. That was the basis of something I said in late 2025, that a lot of what you pay for as NMN takes the niacin road. It is true. It is also true that the NAD+ still gets built, which is what every trial below measured.

Why Not Just Take Niacin
The Cheapest Route Comes With a Flush

Niacin works. In a trial that pushed it to 750 to 1,000 mg a day, blood NAD+ rose as much as eightfold and muscle NAD+ in the patients came back up to the level of healthy controls, and it costs almost nothing. The catch is the flush: at the doses that move NAD+, niacin triggers a prostaglandin release in the skin that turns you red, hot and itchy for half an hour, and some people in that trial also trended toward anemia. Plenty of guys try it and quit. NMN is the route that gets you to the same place without the flush, and that is the whole reason it is in this bottle instead of niacin.

What I Have Said About This
NAD+ Is the Waste of Money, and the Precursor Is the Buy

If you have followed me for a while you have heard me on this more than once, so here it is with dates. In November 2025 I laid out the three ways to raise NAD+ and said niacin was the cheapest proven one, with the gut turning a lot of NMN into niacin anyway, and the gut part still holds. In March 2026 I said NAD+ itself, the injections and the NAD+ supplements, is a waste of money, because the molecule is too big to get into a cell and your body breaks it down into precursors before it can use any of it, and because a man whose underlying problem is consumption, meaning inflammation and damaged cells running CD38 hot, needs to fix that before he buys fuel. I still say both of those. By August 2026 I had read the dose trials on this page and watched what my own guys got from injections, and I landed where I am now: buy the precursor, and buy the one you will stay on. Niacin is the cheapest precursor and the flush is why people quit it. NMN is the one that gets taken every day.

What the Trials Show
Blood NAD+
Up in Every Trial That Measured It

The largest dose trial randomized 80 healthy adults between 40 and 65 to placebo or 300, 600 or 900 mg of NMN a day for 60 days. Blood NAD+ was up at every dose by day 30 and day 60, and the six-minute walk distance improved at every dose compared to placebo, with the 600 and 900 mg groups walking farthest and 900 adding nothing over 600. In 30 healthy adults, 250 mg a day for 12 weeks raised whole-blood NAD+ with no adverse effects. In healthy older men, 250 mg a day for 6 to 12 weeks raised NAD+ and produced small, nominally significant gains in gait speed and grip that the authors themselves said need a bigger trial.

Pooled, a 2024 meta-analysis of 12 trials and 513 people found the NAD+ rise every time and then said something the supplement industry does not repeat: most of the clinical outcomes it looked at were no different from placebo, and the field is exaggerating. You are reading that here because the honest case for NMN is the NAD+ rise plus a handful of functional results, and that case is strong enough without inflating it.

Performance
Aerobic Capacity in Trained Runners, Afternoon Dosing in Older Adults

The trial closest to a man who trains is the one in 48 amateur runners training five to six days a week for six weeks on placebo or 300, 600 or 1,200 mg. Oxygen uptake and the power at both ventilatory thresholds rose more in the 600 and 1,200 mg groups than on placebo, which is an endurance result, not a strength one. In 108 older adults on 250 mg for 12 weeks, the time of day mattered: afternoon dosing improved the five-times sit-to-stand and cut drowsiness, and morning dosing did not separate from placebo. And a 2025 meta-analysis of older adults found no effect on muscle mass, grip or gait speed across the trials it pooled, while another found a gait-speed gain. Read those together: the case is for energy and endurance, and it is not a case for muscle.

What It Does Not Do
No Effect on Blood Sugar or Lipids in Healthy Adults, and No Data on Top of Injections

In healthy adults, eight trials pooled together found no change in fasting glucose, insulin, HbA1c or lipids. The one trial that did move insulin sensitivity was in 25 postmenopausal women with prediabetes, and a published comment on it points out the placebo group started with more than twice the liver fat, so hold that result loosely. And no trial anywhere has tested oral NMN on top of injectable NAD+, so if you are already injecting, nobody can tell you this adds anything, including me.

Safety
Up to 1,250 mg a Day Without a Signal

Thirty-one healthy men and women took 1,250 mg of beta-NMN once a day for four weeks with no changes beyond normal variation in blood work, urine or body composition. A 2026 meta-analysis of 15 trials at 250 to 2,000 mg a day for up to 24 weeks found no increase in adverse events of any kind and no change in liver enzymes. Two capsules of this is 500 mg.

This is for the man whose sleep, training and food are handled and whose energy still fades in the afternoon, whose endurance is not what it was, and who wants the precursor route without a needle and without the niacin flush. Where It Adds Little
If the Drain Is Wide Open, Fix That First

If you are 25 with good sleep and a clean diet, your NAD+ is probably fine and you will not feel this. If you are carrying a lot of inflammation and your training and recovery are a mess, CD38 is burning NAD+ faster than any precursor can supply it, and the money goes further on the foundation. And if you are already injecting NAD+, see above: there is no data that this adds anything on top.

How to Use
Serving

2 capsules daily with water, preferably with a meal, the way the label reads. One serving is 500 mg, which sits between the 250 mg trials and the 600 mg arm that did best in the 80-person dose trial.

Timing

Take it with lunch. In the one trial that tested time of day, afternoon dosing improved lower-body function and drowsiness in older adults and morning dosing did not. If it keeps you up when you take it late, move it earlier.

Consistency

The trials ran 6 to 12 weeks, and blood NAD+ was up by day 30. Give it the 12 weeks the trials gave it, and then judge it by what you notice, because that is the only measurement you have.

Stacking

NAD+ carries electrons to the chain and CoQ10 carries them across it, so the two sit next to each other in my stack. Methylene Blue routes around the damaged stations of the same chain. Creatine recycles the ATP that all of this produces. The Daily Multivitamin carries niacinamide at a vitamin dose, which is nowhere near the amount that moves NAD+, so there is nothing to double count.

What to Expect

Weeks 1 to 4: Blood NAD+ rising. You will not feel that. What some men report first is the afternoon fade shortening, which lines up with the drowsiness result in the afternoon-dosing trial.

Weeks 4 to 12: The endurance side, if it shows, shows here: longer before you gas out on conditioning work, which is the walk-distance and ventilatory-threshold result. Strength is not what this is for. And a share of men feel nothing, which the meta-analysis predicts, so run the 12 weeks and decide from your own data, not from this page.
